Due to continued growth and expansion, we are seeking a dynamic individual to assume the role of Care Manager at Qualicare Burlington. As a Care Manager within our Burlington Office, you will be accountable and responsible for planning, directing, and evaluating client and staff; implementing the standards of care; and the development of staff personnel.
As a Care Manager you will provide leadership and direction in all aspects of client care. The Care Manager will function and participate in the development, implementation, monitoring and evaluation of client programs and services while upholding Qualicare brand standards. The Care Manager will assist in the delivery of home care services including client assessments, health education/counselling and follow-up care.
ROLE REQUIREMENTS:
As a Care Manager, you will:
- Implements administrative and client care policies and practices for the Qualicare location in compliance with state/provincial regulations.
- Consults with supervisors and staff, individually or in groups, regarding clients, families, special programs and service programs
- Oversees client care management processes including intake-to-client conversions, case set-up, customer service, and converting inquiries to service provision.
- Directs the implementation of service goals and objectives
- Develops standards that ensure safe and effective services to clients and families.
- Oversees the implementation and evaluation of client care services inclusive of regulatory licensure, certification criteria and accreditation standards.
- Collaborates in the development of an effective and efficient client care documentation system including statistical compilation and analysis relative to cost and staff service delivery.
- Achieves/exceeds client satisfaction and investigates issues and follows up promptly.
- Supervises and manages overall performance of staff including compensation.
- Assesses appropriate staff supervision during all service hours and directs staff in performance of their duties.
- Directs and supervises professional and auxiliary personnel rendering client care services which include scheduling staff for both new and existing client engagements.
- Ensures staff development including caregiver orientation, in-service education, continuing education, and evaluation of staff
- Achieves/exceeds employee satisfaction and investigates issues and follows up promptly.
- Participates in the review, analysis and appraisal of the effectiveness of the Qualicare program.
- Directs and monitors organizational performance improvement activities.
- Coordinates with the Quality Improvement Manager in the timely implementation of corrective action plans and controls.
- Oversees organizational risk management program and ensures compliance with federal and provincial/state legislative requirements in areas such as health, employment, and occupational health & safety.
- Evaluates service policies and functions which affect the Qualicare location, and recommends proposals for changes
- Evaluates the performance of the individuals in the program in relation to established standards and the individual's professional development.
- Implements utilization review activities.
- Participates in selective conferences and workshops to promote knowledge in community health trends, efficiency in job performance and overall professional growth
- Establishes and evaluates performance, personal goals and objectives for advancement in planning for personal growth
- Perform other duties as assigned
- Registered Practical Nurse/Registered Nurse with current unencumbered licensure to practice professional nursing in the state/province in which the care is being provided.
- Clinical experience in home health or related health care organization.
- Three to five years of direct employee supervision is preferred
- Possess exceptional nursing assessment skills
- Proven ability to build quality and implement improvements with the operation
- Excellent interpersonal skills and analytic/critical thinker
- Excellent customer service skills and ability to deal tactfully with clients, staff and community
- Policy development is an asset.
- Understands provincial/state and federal regulatory compliance
